The Position:

We are seeking a highly motivated and experienced Care Manager. You will provide all care management activities and ensure the safety, wellbeing and positive outcomes for clients receiving a government-funded Home Care Package.  

You will be responsible for:

  • Supporting clients to maintain their optimal level of independence and safety at home.
  • Providing ongoing client assessment and monitoring to ensure care and services meet their individual needs, preferences and goals.
  • Developing and maintaining client individualised budgets.
  • Managing care coordination.
  • Development and review of care and service plans in consultation with clients and their chosen representatives.


The position has a level of professional accountability and responsibility in line with SereneCare policies and procedures and legislative requirements of the Home Care Package Program.

You will play an integral role in the SereneCare team, providing personalised services with a high level of compassion, loyalty, and commitment, ensuring our clients live a full, dignified, and serene life.

Key Selection Criteria:

  • Qualification in Care Management, or undergraduate qualification in Nursing/Allied Health/Social Work.
  • Demonstrated experience in care management and knowledge of Home Care Packages.
  • Demonstrated experience completing initial and annual client assessments and care and service planning within a community setting.
  • Contemporary generalist aged care knowledge and experience.

  • You have completed a Certificate III or IV in Aged Care or are studying or have completed a nursing course.
  • You hold a current First Aid Certificate.
  • You hold a current National Police Check Certificate (valid within 3 months of commencement with SereneCare)
  • You hold a valid and current Australian drivers’ licence, have a road worthy vehicle, registration and comprehensive insurance.
  • You are a confident and safe driver and able to drive in Melbourne traffic.
