The Position:

We are seeking a highly motivated and enthusiastic Care Manager. You will provide care management activities and ensure the safety, wellbeing and positive outcomes for clients receiving a government-funded Home Care Package.  Opportunities also exist to maintain your clinical skills through support and delivery of services to our client base.

You will be responsible for:

  • Supporting clients to maintain their optimal level of independence and safety at home.
  • Providing ongoing client assessment and monitoring to ensure care and services meet their individual needs, preferences and goals.
  • Developing and maintaining client individualised budgets.
  • Providing clinical support and expertise as required.
  • Development and review of care and service plans in consultation with clients and their chosen representatives.

                                                                                                                                                                                                                                      The position has a level of professional accountability and responsibility in line with SereneCare policies and procedures and legislative requirements of the Home Care Package Program. You will play an integral role in the SereneCare team, providing personalised services and clinical support with a high level of compassion, loyalty, and commitment.

                                                                                                                                                                                                                                Key Selection Criteria:

  • Tertiary qualification in Nursing, Allied Health, Social Work or related field.
  • In the absence of tertiary qualifications, demonstrated experience in care management and knowledge of Home Care Packages.
  • Demonstrated experience completing client assessments and care and service planning within a community setting.

The Position

Your duties and responsibilities as a Domestic Assistant will vary. We tailor our home care services to meet the specific needs and individual preferences of each client, so your tasks may include:

  • dusting, vacuuming, and mopping
  • cleaning bathrooms and kitchens
  • making beds and changing bed linen
  • hanging, folding, and ironing laundry
  • shopping or assistance with errands.


You will be required to travel between client homes and service multiple clients during a typical working day (9.00am-5.00pm on weekdays). Actual work hours are flexible, but your consistent and regular commitment is important to our clients and us.

 

Essential Requirements

  • The right to work in Australia
  • A current National Police Certificate
  • A current Australian Driver’s Licence
  • A registered, roadworthy vehicle with fully comprehensive car insurance
  • Up to date COVID and flu vaccinations
  • A smartphone

 

Desirable

  • Previous experience as a domestic assistant/cleaner
  • A current Level 2 First Aid (including CPR) Certificate
  • Fluency in a second language other than English

  • You have completed a Certificate III or IV in Aged Care or are studying or have completed a nursing course.
  • You hold a current First Aid Certificate.
  • You hold a current National Police Check Certificate (valid within 3 months of commencement with SereneCare)
  • You hold a valid and current Australian drivers’ licence, have a road worthy vehicle, registration and comprehensive insurance.
  • You are a confident and safe driver and able to drive in Melbourne traffic.
